I can't remember the game's name for the life of me. It's a game on the PS2 where you play as a little girl. There's this one part where you're in a building and you go up stairs as you're running away from a demon thing. There's a little window on one of the bottom corners of the screen that shows the view of the demon-girl chasing you. In order to get rid of the demon-girl you have to quickly run down stairs by a fire extinguisher and pull the pin making it leak gas-like stuff everywhere. Doing so sort of 'stuns' the demon-girl then you must proceed to beat the crap out of the demon with an umbrella. Does anyone have a clue about which game I'm talking about?
I don't have a picture but those floating women in Cry of Fear that make you commit suicide and if you get to close a baby pops out of her stomach with a knife or whatever. Shit was scary when you weren't expecting it. That and the hanging ones that scream and fall from the trees.

The one part where they really got me though, is when I blew one of their legs off for the first time with a shotgun. I sat there and watched in squirm on the ground while I lined up my next shot, AND THE FUCKER JUMPED ON ME FROM THE GROUND AND STARTED CHEWING ON MY NECK. I damn near had a fucking heart attack. My sister had a heart attack and she was in the next room.

Actually, my favorite thing about SH4 was how they made the apartment a safe haven. It even regened your heath. Then it completely fucked you over later in the game.

Where was it in Yume Nikki that like...I can't remember what you'd do, but the screen would go all black, and a face would slowly come into focus on the screen and there'd be this absolutely terrifying howl in the background? All I remember was Madotsuki woke up immediately after and that was the last time I played Yume Nikki.

Don't necessarily expect it to be scary though. It had its moments, certainly, but if you're good at RPGs you can pretty easily become a nigh-untouchable murder machine even on hard.
Scariest part of the game for me. Running around a bunch of interconnected ambulances not knowing where he's hiding. At one point I turned a corner and he was standing RIGHT FUCKING THERE towering over me (I was playing as Con). I ran like a bitch.
...Then I shot his afro and the fight was over.
Also, the high school's atmosphere was creepy as hell, especially the fight with Nightmare.

This little fella from SILENT HILL (1 XD) are the scariest creature i've ever met. Little horrid children that move slowly, attracted by your light and trying to stab you whit their little fnives. VERY SCARY!

Second game also has a broken neck ghost but she's less fucked up. There is also a mangled ghost who kind of resembles a spider but she's more annoying than scary and it is possibly to completely bypass her.
Not the scariest but since I just played it for the first time a few days ago. The Werewolf in VTMB. Holy fuck I was not ready for that thing. Having it crashing through shit chasing me scared the fucked out of me.

Secret final boss of one of the SNES Kirby games, if I'm not mistaken. The pink monster thing is the Ultimate Chimera from Mother 3. The bipedal alligator is from one of the DK games. I can't remember which. And the knight guy is Zelgius, from Fire Emblem: Path of Radiance. The hands could be zombies from Resident Evil. I don't really know.

Scariest doesn't mean the best horror though. Lots of the most scary games don't understand the need for cooldown periods where you get a (sometimes false) sense of security.
Being scared all the time is hard work. I couldn't play Project Zero for more than 30-45 minutes at a time because it'd wear me down.
